What’s inside the nps survey template

This NPS survey template asks the standard 0–10 "how likely are you to recommend us?" question, then a follow-up that fits the score: what to fix for detractors, what would make it a 10 for passives, and what they value for promoters. Customer IDs and plan names travel in the link, so every score has context.

Change any question, theme or ending in Pulseware, then share it by link, QR code or embed.

  • 4 questions only appear when an earlier answer calls for them.
  • A logic rule skips ahead or ends early, based on the answers.
  • 2 thank-you screens, picked by the answers: “Thank you for the vote of confidence”, “Thanks for your feedback”.
  • Hidden fields filled from the link: company, customer_id, email, plan, segment.
  1. 1 NPS: How likely are you to recommend us to a friend or colleague?
  2. 2 Long answer: What is the main reason for your score?
  3. 3 Long answer: What would we need to fix for you to rate us higher? (optional) (when needed)
  4. 4 Long answer: What one thing would make it a 10? (optional) (when needed)
  5. 5 Multiple choice: What do you value most? (optional) (when needed)
  6. 6 Yes / No: May we contact you about your answer? (optional)
  7. 7 Email: Email (when needed)
