This anonymous feedback form template lets a team, a class, a community or your customers tell you what they think without being identified. Anonymous mode is on, so no IP address, device or location is stored with the answers. People pick a topic, give a rating, and say what works and what should change. Share the link or a QR code and it works as an online suggestion box.

Why this template works
- Anonymous mode is already on, and the first lines tell people what is kept and what is left out.
- Concerns get a seriousness question, and serious ones see who looks at them and how fast.

Questions about this template
Is the form really anonymous?
Yes, unless someone identifies themselves in an answer. With anonymous mode on, Pulseware keeps the answers, the time they were sent and how long the form took. It keeps no name, email, IP address, device, browser, location or referring site, and visits are never linked to a response.
Can the form owner see IP addresses?
No. An anonymous form saves no IP address and no IP hash with a response, and neither appears in the dashboard, exports or integrations. The address is read only while the request is handled, to stop floods of spam, and it is never stored with an answer.
What could still identify someone?
Their own words. A project name or a date can point to one person in a small group, and the optional reply question asks for contact details. The form says so before each of those. Delete the reply questions if nobody should ever be identifiable.
Can I use it as an anonymous suggestion box?
Yes. Leave the form open, put the link on your intranet or the QR code on a poster, and people can send as many suggestions as they like. Duplicate prevention is off in this template.
Can I reply to anonymous feedback?
Only when the person leaves contact details in the optional reply question. For everyone else, reply in public: share a summary of what you heard and what you will do about it.
